Hospital and health system leaders in case management across New Jersey participated in a July 28 webinar (presentation) to learn how the Program of All-Inclusive Care for the Elderly (PACE) partners and collaborates with them to serve their communities. Kicking off the webinar was Deborah Visconi, President & CEO of Bergen New Bridge Medical Center, who described PACE as managing lives, rather than managing benefits as other health plans do. She focused her remarks on how PACE is instrumental in coordinating care following a hospital discharge, especially for complex, nursing home eligible people who are living in the community. The Association discussed the unique features of PACE, including that it is both provider of services and payor and there is daily touchpoints with all PACE participants with the interdisciplinary team. Transportation is a foundational aspect of PACE which supports participants in coming to the PACE center, keeping medical appointments in the community and staying engaged in life with their friends and families.
All New Jersey counties are slated to have an operating PACE organization by 2028; at this time 13 counties have an operational PACE program. More information is available on this site or by reaching out to admin@njpaceassoc.org.
