How Does PACE Work?
PACE is a holistic and comprehensive health and social service program dedicated to keeping you thriving as you age. A team of healthcare professionals, social workers, home care staff, and others create a plan tailored to your needs and preferences.
PACE will cover your care, as well as anything you need to live a healthy life, including prescriptions, medically necessary care, transportation, social activities and more.

Who is Eligible for PACE?
To qualify for participation in PACE, you must:
Be at least 55 years of age
Live in a PACE Service Area
Meet NJ’s Criteria for Nursing Home Care
Be Living Safely at Home at the Time of Enrollment
